Kaiser - Therapy

Please complete the PBH online initial form and then call Kaiser to complete the screening process. After you are authorized for services, Kaiser will assign you to a clinician who has available appointments. If a clinician has already agreed to provide services, you will need to provide Kaiser with the therapist name.

If you are a Kaiser member, please do not complete the online initial form. Instead, contact Kaiser to complete the screening process. After you are authorized for services, Kaiser will provide us with an authorization number. We will then contact you.
